You can try explaining the patient politely that you took a little long on a particular patient because of some unavoidable circumstances and may be apologize for the inconvenience caused to the patient. And even after repeatedly explaining the patient he/she doesnt budge, then you must start being stern and put across your point strongly. Even then if the patient doesnt listen, then i think you must ask her to see some other dentist.

In severe circumstances the doctor must intervene
One must remember that people and social norms expect doctors to be polite and sophisticated so one must sttempt to live up to that
