     Here are some things that u can do:

    * Start her on antibiotics and NSAIDs for 7 days

    * Check dryness of canals. 

    * If there is too much of infection then you can irrigate the canals ( and the periapex) with injection gentamycin. I do it in very rare cases

    * Drain the abscess.

    Most of the times a proper biomech supported by antibiotic cover does the job. RCF should wait till the lesion heals up.

    Post IOPA if possible.

     Sir,

     we put her on NSAIDs and antibiotics, canal was dressed in ca hydroxide..today the patient came, swelling reduced, but pain is still there and that sinus like opening is gone

    So that s a good sign…now we have put IODOFORM in the canals

    Il put the IOPA soon… there is slight infection in the furcation area
